Incident Investigation Checklist

If materials/structures are involved ask if they are shown on job plans and if so take a photo of the area of the plan where their placement, etc. is shown. Also get a photo of the revision date on the plans in use.

Request a TEI Tool Watch report for the involved tool(s) if applicable.

Please note that following any incident/accident/near miss the THA’s (and any related paperwork) involved should be reviewed and the “situation” noted in Section 7 – Post Job Review and new THA’s created, reviewed and utilized ensuring the deficiency which was noted in Section 7 of the previous THA’s is properly addressed. For good practice, obtain copies of the “New” THA’s for inclusion in the investigation and/or root cause analysis documenting the actions taken to prevent re-occurrence. If outdated policies, procedures, forms were being used by the involved parties, collect these also.
